県民健康調査（けんみん　けんこう　ちょうさ）=Medical Survey for Fukushima Citizens=
If you receive an envelope from Fukushima Prefectural Government, it is Medical
Questionnaire to measure how much radiation you were exposed between March 11
and July 11. If you have difficulty reading and filling in the questionnaire, please refer to the
English translation and fill in the Japanese format,
